By using any of the services, products and information provided by Sea to Sky Nutrition Services the user/reader will hereby not hold Marnie Melsted, RD(t) in any way responsible for any consequences resulting from the use of any services, products and information provided on seatoskynutition.com or by Marnie Melsted, RD(t).
The user/reader, hereby grants permission for Marnie Melsted, RD(t) the right to collect and use my person information relevant to my nutritional treatment and counselling. I acknowledge that any information collected will be held in strict confidence. Furthermore, I acknowledge the information provided to me be used strictly for my personal dietary needs and is not suitable for any other individual. Therefore, information will not be transferred, copied or sold to another person.
I acknowledge that Marnie Melsted, RD(t) does not provide or make any medical diagnosis and any medical concerns should be discussed with a physician. I understand that it is important for me to inform either my physician or Marnie Melsted, RD(t), of any changes I make in the application of my diet. It is my responsibility to report any side effects or problems immediately to a physician or Marnie Melsted, RD(t).
Marnie Melsted, RD(t), in turn, promises to practice dietetics in accordance with the regulations as set out by The College of Dietitians of British Columbia and Dietitians of Canada as well as kept all personal information secure and confidential.
